After scheduling your appointment, you will receive an email instructing you to complete intake paperwork and consent forms. It is crucial that these forms are completed at least 24 hours before your appointment to avoid cancellation. As stated above, you can utilize your insurance to cover consultation costs. If using insurance, there will be a section on the intake paperwork to add your insurance information.
